Required Forms for 2017 - 2018

Forms required for all students: Class of 2018, Class of 2019, Class of 2020, Class of 2021.

Medical forms required for all new students (Class of 2021 and transfers) and all student athletes.

Please print TAPPS Physical Examination & Medical History Forms and ask your doctor to complete all sections, including the vision, hearing and spinal screening. Submit a hard copy to IWA by Monday, July 31st.

A copy of immunization records is required for all new students (Class of 2021 and transfers).

8 th - 12 th grade State Law immunization requirements:

  • TdaP vaccine: 1 dose when 10 years have passed since the last dose of a tetanus-diphtheria-containing vaccine. Td is acceptable in place of TdaP if a medical contraindication to pertussis exists.
  • Meningococcal: 1 dose for grades 7 - 11 ; not required for 12 th grade.Varicella: 2 doses required for grades 7 - 11 t; 1 dose for 12 th grade; 1 st dose must be on or after 1 st birthday. For any student who receives the 1 st dose on or after 13 years of age, 2 doses are required.
  • MMR: 2 doses of a measles-containing vaccine and 1 dose each of rubella and mumps.

Please submit a hard copy to IWA by Monday, July 31st.

 

Scheduled and “As Needed” Medication Permission Form

This form is required for any medications taken at school including medications such as Tylenol. Please print and ask your doctor to complete. Submit a hard copy and an unopened container of the medication to be administered to IWA.

iPad Program

Incarnate Word Academy has a one-to-one iPad program in which all freshmen are issued a 128GB 5th-generation iPad for the school year.

iPad Distribution

iPad distribution will take place for new students during Freshman/Transfer Orientation on August 10, 2017.

In order for freshmen and incoming transfer students to receive an iPad on this date, each student must bring the following to pick up: 

  • Apple ID
    Each iPad must have a free Apple ID associated with it. Only one Apple ID may be associated with the iPad and this Apple ID must belong to the student. For more information about Apple IDs, visit the Apple ID Support webpage. To create an Apple ID, visit the My Apple ID webpage. Parents are encouraged to setup Family Sharing. New students should create an Apple ID before picking up their iPad.

  • Approved iPad 5th Generation Case (FRESHMEN ONLY)

    Since the new iPad being issued to students is still new, there are limited options available for purchasing a case. IWA is able to procure iPad cases from Apple directly at discounted prices. Below is a list of approved cases. The STM Dux is the only one that is available for purchase on Amazon and other retailers.

    Parents may order a case to be purchased through the school via an online Google Form. Freshmen who purchase the iPad case through IWA will receive the case when issued their iPad. The purchase price will be added to your IWA Smart Tuition account. iPad case orders are due by July 10, 2017.
